When you submit this form you are sending your information to Hertfordshire County Council (as the Data controller) the data on this form is being gathered for the purpose of providing a crisis intervention service to Hertfordshire residents.

The information you give us will be held by the web team of Hertfordshire County Council and will only be used to share with POhWER, who are the providers of this service.

POhWER may then make a referral to a third party provider dependent on the service user’s needs including but not exclusive too:

Struggling to cope – Samaritans

Homelessness – Shelter, Herts Young Homeless, North Herts Sanctuary

Abuse & Sexual Abuse – Herts Area Rape Crisis, Domestic Abuse Helpline, Herts Sunflower.

If the service user is in financial crisis they will be assessed on their needs and referrals can be made to food banks, HILS (for food parcels) Clothes and Furniture banks. All financial crisis contacts are referred to Citizens Advice for debt management and advice.

We may also share information with third parties if we are legally obliged to do so, for example if it necessary to safeguard or protect a child.

We may also share information with the police or other agencies if it is necessary for the following purposes:

a) The prevention or detection of crime

b) The apprehension or prosecution of offenders

c) The assessment or collection of any tax or duty or any imposition of a similar nature.
